What to Expect

Thank you for trusting Dr. Porter and the team at Southwest Oral Surgery with your care. We look forward to meeting you!

Please complete your new patient registration here (needs to be done in one sitting and SUBMIT must be entered at the end in order for us to receive it, or complete the printed form and bring it in).

When you arrive at our clinic for your initial appointment, we will scan a photo ID, any insurance cards you have, and the referral from your doctor. You will meet with a dental assistant, who will take any necessary imaging and review the reason for your visit and medical history. Please bring a list of any medications you are currently taking, and the contact information of the prescribing doctor. Dr. Porter will then speak with you, perform a physical exam, review your x-rays, and offer his diagnoses and treatment plan. You are welcome to discuss any questions or concerns you may have with the Dr. at that time.

One of our team members will then meet with you to review the recommended treatment plan, associated financial responsibility, and discuss scheduling for surgery.
